McGill Short Form Pain Questionnaire and Pain Diagram
Printable PDF Version of the Test for Patient Use
The MPQ-SF includes 15 words from
the Sensory (11) and Affective (4)
categories.  The patient is asked to
rate if their pain has each word's
"quality" or not, and if it is present,
rate it as "mild, moderate or severe".
In addition, the patient rates the
overall intensity of pain on a
visual-analogue scale (1 to 10), and
a "present pain intensity" by
choosing an appropriate word (this is
a 0 to 5 scale).

SCORING THE SF-MPQ:
Sensory pain score : This is the
total score of the 11 words
describing the sensory components
of pain severity converted to a
percent.  If the patient chooses
"severe" for all 11 words, he or she
scores "100%".  Choosing "mild" for
all 11 words would yield 11/33 points
or a score of "33%".
Affective pain score:Totals the
VAS: Converted to %.
Pain Rating Index (PRI): Maximum of 45 for the test: = affective and sensory raw
scores before conversion to percent.  The PRI serves as a trending number over time.
